Welcome to Pasadena ENT & Allergy. Our ENT specialists, Dr. Shelley Jaquish and Dr. Timothy Dunlevy, strive to provide you with caring and comprehensive medical evaluations and the most advance treatment options available to resolve your health issues and improve your quality of life.

What to Expect on Your First Visit

To save yourself some time in the office, prior to your visit, please down-load and complete the Adult - New Patient Packet or the Pediatric - New Patient Packet. This will greatly speed-up your check-in process and allows you to see the doctor more quickly.

While our goal is to see patients at their scheduled appointment time, our doctors are surgeons and can occasionally be called into emergency surgery or to see a patient with a medical emergency. If an emergency arises, we will do our best to notify you prior to your appointment, however, this is not always possible. Your patience and understanding is greatly appreciated.

Please bring a photo ID and insurance card to your first appointment

Certain patients may also need to provide a referral form from their primary care provider. Please bring copies of any x-rays taken.

We participate with a wide variety of insurance plans. Please contact us to confirm your insurance carriers participation.

Office Hours

We provide care Monday through Friday from 8:00 a.m. to 4:30 p.m. Patients are seen by appointment only.

In Case of Emergency

If you have an after-hours emergency, please call our office at: 727-341-0551 and our answering service will contact the doctor on call.
