From South St. Petersburg

• Take I-275 north to exit 30 Roosevelt
  Blvd./686-W. Bear left toward Largo/
  Clearwater.
• Merge onto Roosevelt Blvd. N./SR-
  686 W.
• Once on Roosevelt Blvd. N., get into
  the far right lane. This lane becomes a
  right-turn-only lane into the Carillon
  Office Park.
• Turn right onto Lake Carillon Dr.
  At the first light, turn left onto Carillon
  Parkway.
• Follow Carillon Parkway and, just past
  the Raymond James building, you will
  see the St. Anthony’s Outpatient
  Center-Carillon on the left.

From Largo/Pinellas Park

• Take Roosevelt Blvd./SR-686 E.
• Follow Roosevelt Blvd./SR-686 E.,
  which becomes Ulmerton Road/
  SR-688 E.
• Continue east on Ulmerton Road/
  SR-688.
• Turn right onto Carillon Parkway W.
• Turn left onto Carillon Parkway.
• The St. Anthony’s Outpatient Center-
  Carillon is immediately on the left.

From Clearwater

• Take the Bayside Bridge toward Largo/
  St. Pete-Clearwater Airport.
• Turn left onto Roosevelt Blvd./
  SR-686 E.
• Follow Roosevelt Blvd./SR-686 E.,
  which becomes Ulmerton Road/
  SR-688 E.
• Continue east on Ulmerton Road/
  SR-688.
• Turn right onto Carillon Parkway W.
• Turn left onto Carillon Parkway.
• The St. Anthony’s Outpatient Center-
  Carillon is immediately on the left.
  From Tampa
• Take I-275 south over the Howard
  Frankland Bridge.
• Merge onto Ulmerton Road/SR-688 W.
  via exit 31 toward Largo.
• Turn left at the second traffic light after
  exiting the Interstate (the second
  Carillon entrance).
• Turn left onto Carillon Parkway.
• The St. Anthony’s Outpatient Center-
  Carillon is immediately on the left.
